Overview
We are looking for a Therapy Assistant Practitioner to join our Community rehabilitation team in Dorchester. You will work with a team of nurses and therapists to set goals and assist patients to regain their independence. Being a car driver is essential for this role.
Responsibilities
* Carry out holistic assessments and formulate treatment plans.
* Monitor progress against set goals under indirect supervision of a registered therapist.
* Provide rehab‑based care to people in their own homes.
* Complete autonomous assessments of mobility, equipment and patient function following triage from a registered clinician.
* Provide monitoring and follow‑up through clinical assessment and interventions, seeking supervision from the registered clinician.
* Provide advice and rehabilitation programmes for other support staff to carry out.
* Support the team to respond to patients' needs to support hospital discharge, ensuring care needs are met for home stay.
Qualifications & Requirements
* Essential: valid right to work in the UK; visa sponsorship not available.
* Essential: valid car driver licence.
* Essential: Ability to carry out assessments and work under indirect supervision of a registered therapist.
* Employment requires an Enhanced Disclosure and Barring Service (DBS) check; the Trust will cover the cost.
