Band 4 Associate Practitioner - Gravesham Community Hospital, GravesendWe have an exciting opportunity for a Band 4 Associate Practitioner to join our dedicated team on Sapphire Ward at Gravesham Community Hospital.Working under the supervision of a Registered Nurse, you will play a key role in delivering safe, high-quality care to patients in a rehabilitation setting, following national and local priorities, Trust policies, and Nursing & Midwifery Council standards. This role requires NMC registration and is ideal for someone with health or social care experience who is ready to develop their clinical skills, work as part of a supportive multi-disciplinary team, and make a genuine difference to patient outcomes.Main ResponsibilitiesAs a Band 4 Associate Practitioner you will be:Deliver patient care in accordance with Trust policies, procedures, and professional standards as delegated by the Nurse in chargeUndertake technical nursing tasks such as dressings, glucose monitoring, and other delegated clinical proceduresAssist with patient assessments and implement care plans set by the Registered NurseAccurately document all care provided using EMIS and ensure confidentiality is maintained at all timesRecognise and report changes in patient condition promptly to the Nurse in chargeSupport administrative and clerical tasks on the ward, including ordering stock and equipment checksParticipate in audits, surveys, and multi-disciplinary meetingsSupport the induction of new staff and studentsEngage in personal development through training, supervision, and appraisalPromote the safety and wellbeing of patients, staff, and visitors, ensuring a safe working environmentAdhere to information governance, safeguarding, infection control, and health & safety requirements at all timesPlease see job description for full list of responsibilities. Ideal CandidateWe are looking for someone who:Is NMC registered and committed to maintaining professional standardsHas previous experience in a health or social care setting (rehabilitation experience desirable)Can work well within a team but also organise their own workload without direct supervisionHas strong communication skills and a compassionate, patient-focused approachIs flexible to work across a seven-day rotaIs competent in IT systems and willing to learn EMISCan undertake a range of clinical skills, with desirable experience in venepuncture, catheter care, stoma care, and wound managementCan assess patient needs and implement care under the direction of a Registered NurseCan cope with occasional exposure to distressing or emotional situations while maintaining professionalismIs committed to personal and professional developmentAbout the CompanyWe change lives by transforming health and care.Established in 2006, we are one of the UK's leading independent providers of community health and care services, working with health and care commissioners and communities to transform services with a focus on experience, efficiency and improved outcomes.
