We are exciting to offer a rewarding opportunity within the Inverness district flow hub for a band 4 Health and Social Care Coordinator who will be part of the Inverness Integrated team.
The Health and Social Care Coordinator plays an integral role within the Integrated team through supporting the provision of person‑centred, safe, and effective delivery of care. The role involves managing referrals from professionals, patients, and members of the public; collating information from a variety of sources to support early multidisciplinary team decision making so that delivery of care and services can be prioritised and utilised appropriately.
We are seeking someone who has excellent communication, interpersonal, and organisational skills with a health and social care qualification or experience in the sector and demonstrates a good understanding of how health and social care is delivered. Competence with computer systems and attention to detail for updating accurate record‑keeping are essential.
