Job overview
Pennine Care is working towards initiating a new community rehabilitation service to maintain the continuity of rehabilitation support to patients in the community. Over the last 5 years, there has been notable development of a rehabilitation pathway from inpatient rehabilitation to 24-hour support accommodation. While most patients will be discharged to their own homes, some will require ongoing rehabilitation in familiar environments. All patients will be under CPA, and the principal duties of the post holder will include: senior clinical leadership of the CRT service and responsibility for the medical care of patients under the team.
The CRT will be a fully integrated team with an expected workload of around 80 patients by the end of year 3. A key aspect of the CRT model is intensive in-reach work while patients are on the ward, aiming to build trusting relationships prior to discharge and ensure continuity of care, facilitating smooth transitions into the community and promoting independence. The work involves proactive engagement and review of pharmacological and psychosocial treatments, with a strong focus on social, educational, and occupational recovery, aiming for maximum symptomatic and functional recovery and relapse prevention.
Main duties of the job
* Provide clinical leadership for the team, including direct assessments and consultations.
* Lead in weekly MDT care planning and risk management meetings.
* Attend and report for Mental Health Tribunals and Managers hearings as Responsible Clinician for clients under Supervised Community Treatment Orders.
* Ensure compliance with legal obligations such as the Mental Health Act.
* Manage caseloads actively, reviewing patients in supported accommodations, care homes, or their own flats; outpatient clinics may be developed as the team expands, requiring travel.
* Handle physical health issues, including six-month physical reviews, blood investigations, ECGs, liaising with GPs and specialists. Some duties may be delegated to an advanced nurse practitioner, with annual review of the junior doctor need.

* Low secure units at Prospect Place, Birch Hill Hospital, Rochdale, with various specialized beds.
* Tatton unit, a long-term low secure unit at Tameside General Hospital.

Person specification
Essential and desirable
* Full registration
* MBBS or equivalent
* AC status or ability to achieve within 3 months
* Excellent clinical skills with a bio-psycho-social perspective and broad medical knowledge
* Valid UK driving licence
* MRCPsych or equivalent recognized by the Royal College of Psychiatrists
* Experience in rehabilitation or forensic psychiatry
* Experience in conducting audits and participating in the Trust's audit program
* Extensive experience in assessing and managing clinical risk
* In good standing with GMC regarding warnings and conditions on practice
