Job overview

Due to the expansion of our current services, the Community Diabetes Team is seeking an experienced Diabetes Specialist Nurse to join our team. We are inviting applications for the above position to support the Tower Hamlets Community Diabetes Service, based at Mile End Hospital.

As part of a dedicated and enthusiastic team of Community Diabetes Nurse Specialists, you will work alongside other healthcare professionals, providing both clinical and educational support to patients with Type 1 and Type 2 diabetes, as well as their carers.

The successful candidate will possess strong time management, organisational, and communication skills, with the ability to manage a clinical caseload and work autonomously. We are committed to supporting your personal development and offer ongoing educational opportunities.

At Bart’s Health Community Diabetes Service, we deliver structured education for Type 1 diabetes through the Dose Adjustment for Normal Eating (DAFNE) programme, and for Type 2 diabetes through the X-Pert educational programme. We also provide structured education for individuals at risk of developing diabetes (Pre-Diabetes Education). We are particularly interested in candidates who are passionate about advancing their expertise in diabetes technology.

Bart’s Health is a renowned teaching centre, welcoming medical students from Queen Mary University of London and nursing students from City and Greenwich University.

Main duties of the job

The post is an exciting opportunity to work within the speciality of diabetes, contributing primarily to the delivery of the adult diabetes service at Mile End Diabetes Care Centre and within GP practices in Tower Hamlets.

The post holder will develop specialist skills enabling them to provide management plans, treatment advice, and support for adults with diabetes and their carers.

The post holder will be a clinical and educational resource for patients and health professionals; and will ensure a smooth transition from primary to secondary care occurs in terms of diabetes care.

The post holder will be a good team player always striving to improve the care of people with diabetes.

Qualifications

Essential criteria

1. First level NMC registration
2. Mentorship qualification
3. University diabetes course or equivalent level of knowledge or experience

Desirable criteria

4. Teaching and assessing (ENB998) or equivalent level of knowledge or experience
5. Graduate in nursing or health related studies
6. Organised certificate in counselling

Experience

Essential criteria

7. Minimum 2 years post-registration experience
8. Ability to practice within the scope of the NMC Code of Professional Conduct
9. Awareness of the structure and organisation of the wider NHS
10. Experience of supervision of junior staff / nursing students

Desirable criteria

11. Participation in Nursing Research and Audit
12. Experience in the management of diabetes in the community or primary care or of working on a diabetes ward
13. Experience of patient education to support self-management

Skills

Essential criteria

14. Is able to demonstrate understanding and empathy relating to the challenges faced by someone living with diabetes
15. Demonstrates well developed verbal and written communication skills
16. Is computer literate
17. Able to demonstrate effective change management skills
18. Demonstrates excellent interpersonal skills
19. Able to work autonomously as well as a member of the team
20. Can demonstrate teaching / education skills in daily practice
