Role: Care Coordinator
Location: Bedfordshire
****MUST DRIVE AND HAVE OWN CAR****
OFFICE HOURS IN BEDFORD + MUST BE ABLE TO TRAVEL ACROSS BEDFORDSHIRE
Mega Nursing & Care is seeking a dedicated and compassionate Care Coordinator to join our expanding team. The successful candidate will play a crucial role in providing exceptional care and support to individuals within our community.

Package
Salary: £14.28 - £15.02 per hour
Bi-Weekly Pay
Pay Advances - (Up to £1000 which is paid back with no interest)
Mileage contribution
Car Maintenance Scheme - (Get up to £100 each year for any car repairs, MOT and Servicing)

What you will be doing:
Client Assessments and Reviews: Conducting assessments with new service users and conducting regular reviews with existing clients.
Support Plans and Documentation: Preparing support plans, care plans, and all necessary documents for service users' folders, ensuring accuracy and compliance.
Stakeholder Communication: Building and maintaining relationships with company stakeholders, hospital teams, social workers, doctors, and nurses.
Initiating and Managing Care Packages: Starting and overseeing new care packages.
Rota Management: Preparing the rota every two weeks.
Training and Supervision: Training senior support workers and key colleagues, allocating and supervising their work and development.
Liaison with Social Workers: Liaising with social workers regarding current service users.
Issue Handling and Reporting: Handling issues and concerns, collaborating with the branch manager and social workers to resolve challenging situations. Reporting to the branch manager.
Covering Calls and On-Call Duties: Providing coverage for care calls and participating in on-call duties as needed.
Continuous Development: Maintaining current skills through regular training for personal and professional development.
Training Assistance: Assisting with induction, and training of new staff.
Spot Checks and Supervisions: Conducting regular spot checks and supervisions to ensure service quality.
Staff Meetings Participation: Participating in and assisting with staff meetings.
Professional Representation: Representing the company in a professional manner in all communications.
Risk Assessments: Carrying out risk assessments under Health and Safety requirements.
Satisfaction Surveys and Staff Performance Monitoring: Assisting with satisfaction surveys and monitoring staff performance.
Confidentiality and Data Protection: Maintaining confidentiality and adhering to data protection guidelines.
Principles of Mental Capacity Act: Ensuring the principles of the Mental Capacity Act are applied to all service users.
Covering Care Calls Responsibilities: When covering care calls, performing the responsibilities of Community Support Workers, including providing care according to individually tailored care plans, ensuring timely medication administration, assisting with mobility and daily activities, preparing meals, and offering companionship.
Issue Reporting and Documentation: Reporting any issues and concerns to senior staff members. Completing all relevant forms and reports, including MAR sheets, prompt sheets, incident logs, and other necessary documents.
Consent and Ad-Hoc Duties: Providing care with service users' consent and completing ad-hoc duties as requested by the branch manager.
Requirements:
At least 1 year experience as Care Coordinator (Domiciliary Care)
At least Level 2 in Health & Social Care
Must have full valid UK Driving Licence
Must be able to use own car for work-purposes (Including having Business Insurance)
Working Times:
Monday - Friday + Every Other Weekend (between 7am - 10pm)
Full-Time Only
Must be able to travel to the following areas if required:
Bedfordshire
