Hours
Work 30 hours across 4 days, from 8am to 4pm, ideally on Fridays and Mondays, with flexible arrangements for other days. No weekend work.
This is a full-time position with an annual salary of up to £52,507 per annum FTE, depending on experience.

Main Duties
As a Long Term Conditions Nurse, your responsibilities will include:
* Ensuring comprehensive handovers when patients transfer between wings or care settings.
* Identifying immediate health needs and coordinating external services or prescriptions.
* Facilitating collaboration between agencies to deliver complex care packages, working closely with in-house healthcare services and prison departments such as Pharmacy, Education, Offender Management, Substance Misuse, and Physical Education.
* Addressing improvement recommendations from CQC and HMPP inspections proactively.
* Providing continuity of care across health and social care agencies, improving communication between courts, custody, and community services.
* Enhancing MDT working across the prison and healthcare teams.

Qualifications
* Registered Nurse - General with relevant NMC registration
* Advanced Nurse Prescriber/Practitioner
* Experience or knowledge in Diabetes or Sexual Health is desirable
* Degree-level education
* Knowledge of health and social care models within the criminal justice system and relevant initiatives
* Understanding of Evidence-Based Practice and Clinical Governance
* Experience working with individuals with complex health and social care needs
Experience in hospital, community, or prison environments is advantageous but not essential.
