The role will provide a responsive, flexible and high-quality community specialist palliative care service to patients and those important to them. You will;
* Provide specialist nursing care to patients in the last weeks/days of life.
* Complete assessments of patients referred to the Hospice at Home service at any stage in their palliative diagnosis.
* Transfer patients care to other hospice services as appropriate when nursing interventions are no longer required.
* Work independently during weekends and as part of a 7-day rota, including bank holidays.
* Lead and coordinate care planning, ensuring safe, effective, and personalised care.
* Delegate appropriately and support the work of Senior Healthcare Assistants (SHCAs).
* Collaborate closely with the wider multi-disciplinary team (MDT).
We are seeking a highly skilled and experienced nurse who is:
* Ideally experienced in community nursing and/or palliative care.
* Confident working independently, including lone working at weekends.
* Committed to developing professionally within a supportive and values-led environment.
* Skilled at delegating tasks and coordinating care across a team.
* A strong communicator with the ability to work collaboratively with an MDT.
* Driving licence and access to a vehicle are essential for work purposes.
