We are delighted to invite Registered Adult Nurses to join our Staff Bank. We are looking for passionate individuals to support the Unplanned District Nursing service across Bradford, Airedale and Wharfedale. Our Staff Bank opportunities are available to experienced Registered Adult Nurses and Newly Qualified Adult Nurses.
The unplanned service is delivered over a seven-day rota, covering a 24-hour shift pattern. The service supports people in their own homes and in close collaboration with planned care services, anticipatory care teams, general practice colleagues, acute care services and members of the community partnership. The post holder will work collaboratively with all members of the MDT providing urgent care response (UCR) and virtual ward services.
You will be working as part of a team to provide high quality care for people in their own homes, ranging from long term conditions to complex and palliative care. You will do this with the support of agile technology, and specialist services.

Main duties of the job
The post holder is expected to work as part of the community nursing team in unplanned care, under indirect supervision of the district nurse or senior nurse. The post holder is accountable for their own professional actions in line with NMC guidance.
The post holder is expected to plan, implement, monitor, and review programs of health interventions for individuals and populations, which are to be delivered with other colleagues and agencies. They will also liaise closely with the district nurse team leader/senior nurse / wider stakeholders to discuss changes in nursing care, e.g. medication reviews, care plans. Input / update SystmOne as per policy.
The post holder will undertake a variety of nursing interventions for patients with different health needs, this includes Virtual Ward, complex wound management, bowel care, catheter care, medication administration and palliative care including syringe-driver management.
