Clinical Pharmacist – Circle Health Group
Location: Circle Reading Hospital, Reading, United Kingdom
Hours & Type: 37.5 hours per week, Permanent
Salary: Up to £45,000 per year (dependent on experience, training, and qualification)
Overview
We have an opportunity for a Clinical Pharmacist to join our Pharmacy Department. This full‑time role requires covering a flexible shift pattern.
Duties of this role include
* Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
* Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
* Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
* Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
* Promote safe and cost‑effective use of medicines by supporting adherence to local and national policies.
* Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity.
* Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance.
Applicants should meet the following criteria
* A degree in pharmacy (MPharm, BSc, or BPharm).
* GPhC registration as a Pharmacist.
* Previous experience in a clinical hospital pharmacy role.
* Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
* Excellent communication and clinical decision‑making skills.
* Ability to work autonomously and as part of a multidisciplinary team.
* Confident using electronic prescribing and medication administration systems (e.g., EPMA).
