Job Description
This role requires carrying out specialist assessments for individuals referred for major adaptations to their home, due to illness, disability, or palliative conditions. You will manage a complex caseload, including urgent work, and play a key role in enabling people to live safely and independently in their own homes.
Key Responsibilities:Undertake detailed, person-centred assessments to determine the need for major housing adaptations.Manage urgent and complex cases, including those involving palliative care and advanced moving & handling needs.Collaborate effectively within a busy MDT, contributing to holistic intervention planning.Provide expert advice and recommendations in line with legislation, clinical guidance, and best practice.Ensure that all work is completed in a timely, professional, and outcome-focused manner.
Essential Criteria:Degree or Diploma in Occupational Therapy.HCPC registration as an Occupational Therapist.Proven experience in complex Moving & Handling (M&H).Sound knowledge of housing adaptations and equipment provision.Strong assessment, problem-solving, and communication skills.Desirable:Level 4 Assessor Training.Experience working with Disabled Facilities Grants (DFGs) and housing legislation.
