In this role, you'll be at the heart of our ward-based pharmacy service, playing a vital part in ensuring safe and effective medication use. Working closely with pharmacists, your main focus will be supporting medicines reconciliation within 24 hours of patient admission, taking drug histories, and providing medication counselling at both admission and discharge. You'll also help facilitate a smooth and efficient discharge process, ensuring continuity of care.
1. To provide medicines management services to adult wards including compiling accurate medication histories and undertaking medicines reconciliation.
2. To undertake day-to-day supervision of junior pharmacy technicians and pharmacy assistants within the team.
3. To provide training in pharmacy procedures, pharmacy stock control, and electronic prescribing systems, including signing off training logs to all members of staff.
4. To provide straightforward advice on pharmaceutical products and supply.
5. To work closely with other healthcare professionals at ward level.
6. To act as a professional role model for junior pharmacy technicians and pharmacy assistants, actively demonstrating integrity, ownership, and leadership in all aspects of the role.
7. To contribute to the pharmacy service including dispensing and issuing prescriptions via the main dispensary. This will also involve accredited checking of prescriptions.
