The post holder will provide expert clinical expertise and operational management for the Virtual Wards (Hospital @Home) Model within Birmingham Community Healthcare (BCHC) NHS Foundation Trust.
The role involves working across various city-wide settings, including community and inpatient units, with in-reach into secondary care. The aim is to identify and assess patients to prevent avoidable hospital admissions and facilitate early supportive discharge. Collaboration with Secondary and Primary Care, Community Services, Urgent Care Response, Early Intervention Team (EICT), and Specialist Services is essential. The post holder will participate in integrated multidisciplinary teams (MDTs) to provide advice, support, and data to support the Virtual Ward (Hospital @Home) Model.
You will join a community team amidst significant health and social care transformations, focusing on preventing hospital admissions and supporting early discharge.
Main duties of the job
1. Create an environment centered on clients' needs, involving them in decision-making and respecting their dignity, wishes, and beliefs.
2. Safeguard patients by taking emergency action when necessary, reporting risks promptly, and following safeguarding policies.
3. Work within the Virtual Ward Hub, acting as shift lead, triaging new referrals, monitoring patient progress, leading safety huddles, MDTs, and case reflections.

Person Specification
Qualifications/Training
* Registered Nurse Level 1/ Allied Health Professional (AHP), NMC or HCPC registered
* Recent Continuing Professional Development evidence
* Degree-level education or equivalent in nursing or relevant health field
* Non-Medical Prescribing qualifications
* European Certificate in Essential Palliative Care
* Leadership course
* Mentor/Teaching qualification (ENB 998 or equivalent)
Experience
* Experience working in multidisciplinary teams within community or secondary care settings
* Significant post-registration community experience
* Experience conducting clinical audits and holistic assessments of health and social needs
* Experience leading teams, including appraisals
* Experience teaching, training, and mentoring
* Experience managing change
* Knowledge of Health & Safety and Risk Management legislation and application
