Empower Independence. Make a Difference. Liverpool City Council is seeking a dedicated and experienced Social Care Assessor to join our Adult Social Care & Health team on a part-time basis. This is a fantastic opportunity to contribute to a strengths-based, person-centred approach to social care with a focus on reablement. Key Responsibilities: Carry out social care assessments and develop support plans that promote independence Work directly with individuals, carers, families, and partners to coordinate care Manage your own caseload and ensure timely reviews and updates to care plans Apply a reablement-focused approach to all interventions Use Liquid Logic to maintain accurate case records and reports Work collaboratively across multi-disciplinary teams to improve outcomes Essential Criteria: Experience working in a social care setting with a focus on reablement Confident and competent using Liquid Logic case management system Ability to deliver strength-based assessments and outcome-focused care plans Good understanding of adult social care legislation and safeguarding practice Strong communication, organisation, and partnership-working skills Why Join Us?
