Band 3 - Community Support Worker - IMHT Cannock

Midlands Partnership University NHS Foundation Trust offers an opportunity to join our Integrated Mental Health Team based in Cannock and Rugeley.
The Integrated Mental Health Teams work as part of a 'neighbourhood' providing place-based mental health services in partnership with local organisations. Clinicians within the IMHT will offer interventions and treatments aligned with NICE guidelines, with allocations based on the needs of service users, matching clinician skills to these needs.
Main duties of the job
1. You will be responsible for providing planned recovery-focused care working with a defined caseload.
2. Collaborate with other Trust services to ensure seamless care for service users within the Integrated Mental Health Teams.
3. Partner with external agencies to support service-user access to services in the independent and non-statutory sector.
4. Deliver therapeutic interventions as per the care plan to help service users achieve recovery goals, including improving daily activities, social inclusion, meaningful occupation, employment access, and health promotion.
5. Conduct physical health assessments including weight, height, blood pressure, ECG, and phlebotomy.
6. Contribute to planning and delivering therapeutic activities as part of the care programme approach.

Person Specification
Qualifications (Essential)
* NVQ 3 in (Mental) Health and Social Care or equivalent GCSEs/Functional Skills with evidence of numeracy and literacy skills
Experience (Essential)
* Experience working with individuals with non-Dementia mental health needs
Desirable
* Experience working within a multidisciplinary community team
* Experience co-facilitating group interventions
Skills (Essential)
* Knowledge of mental health issues, services, and promotion
* IT proficiency with Windows-based systems
* Ability to work independently, prioritize, and manage competing needs effectively
