As a Regis employee, you’ll be part of our BodeWell Community Care team – working together to enhance the wellbeing of older Australians. BodeWell is an approved provider with the Department of Health, delivering government-funded Home Care Packages, Commonwealth Home Support Program, Private Aged Care, and NDIS services across Queensland and Victoria.
The Intake Officer manages and assesses client referrals, updates service information via the My Aged Care Provider Portal, and completes annual reviews and reassessments as required.
Key Responsibilities
1. Manage client referrals and update service information via the My Aged Care Provider Portal.
2. Complete the BWCC Intake process for CHSP-referred clients.
3. Conduct needs-based, client-centred assessments, and reassess services annually or as needed based on client requirements.
4. Identify clients referred in the CHSP program with an approved or assigned HCP status in My Aged Care and facilitate referral to HCP program with their consent.
5. Provide information about home care packages with BWCC.
6. Identify opportunities for private client referrals.
What you’ll bring
* Organisational and administrative skills with the ability to manage multiple projects with accuracy and attention to detail.
* Excellent communication, customer service, negotiation, problem-solving, and decision-making skills.
* Understanding of issues faced by older adults, including those with dementia, veterans, LGBTI individuals, culturally diverse backgrounds, disabilities, and their carers/families, respecting their social, spiritual, and cultural needs.
* Experience with client database management systems.
* Assessment experience, preferably in Community Services or Healthcare.
* Diploma or relevant tertiary qualifications are desirable but not essential.
* A current state Driver’s License.
To work within Home Care, you will need to provide satisfactory background checks, an NDIS worker screening clearance, and evidence of current flu vaccinations.
