We have an exciting opportunity for a hard‑working and enthusiastic professional to join our Intermediate Care Team in Blantyre LIFE.
In this role, you will be working with people discharged from a hospital setting who require a period of reablement, recovery and assessment to determine any needs. This may involve home visits to support discharge.
In order to be successful in this role, you will have the ability to gather, select and analyse information to support your assessment decisions and identify the underlying cause of any functional difficulty.
You will provide equipment and/or adaptations as a direct solution to the need or refer onto other statutory or voluntary services.
You should work autonomously, meet tight timescales and demonstrate responsibility for your own learning needs.
You should have an HNC in Occupational Therapy Support or be willing to work towards gaining this qualification.
Supervision and peer support will be provided regularly.
Hours: 21 hours per week
