This role is offered on a full-time basis of 37.5 hrs per week. The salary will be on a pro-rata basis.

We are offering an exciting opportunity for Advanced Clinical Practitioner’s in our Acute Medical Assessment Unit. Join our dynamic Team and take your career to the next level. If you're a passionate professional with a deep interest in Acute medicine and Same Day Emergency Care, this role is tailor-made for you.

As a registered professional with a Masters in Advanced Clinical Practice, you will bring with you a wealth of clinical experience. Your expertise will be crucial in assessing, treating, and managing a diverse caseload of adults in our acute Medical Assessment Unit.


Responsibilities

* Develop skills and knowledge to assess individuals and families using person-centred approaches and a range of assessment methods (e.g., history taking, physical examination, ordering and interpreting diagnostic tests, or conducting health needs assessments). This will include independent examination of the patient and obtaining specimens.
* Progress clinical expertise to support decision making, particularly where clinical presentations are not directly covered by standard protocols or where highly complex or unusual aspects require specialised outcomes.
* Take responsibility to autonomously manage a caseload within the specialist area, including management of investigative results to deliver a clear diagnosis, ongoing management plan and discharge arrangements.
* Use a diverse range of knowledge to become competent in evidence‑based therapeutic interventions, including prescribing medication where legally allowed and actively monitoring the effectiveness of therapeutic interventions.
* Be conversant with local and national policies relevant to advanced practice and the role of the advanced clinical practitioner in the specialist area.
* Teach regularly on a range of subjects locally and externally; this will be multi-disciplinary and include the design, development and delivery of specialist training programmes.
* Use a range of IT systems to input data, maintain patient records, and request and interpret diagnostics.
* Demonstrate a sound understanding of clinical governance and risk management issues and, in partnership with the lead advanced clinical practitioner, facilitate the implementation of standards, policies, procedures, competencies and clinical guidelines.
* Support and maintain safety, quality, clinical effectiveness and patient experience, working with clinical leaders to ensure a culture of safe and effective harm-free care, with patients, relatives and carers central to service delivery.
* Utilise expert analytical and critical thinking with clinical skills to ensure that care and services are innovative, contemporaneous and clinically effective.
* Provide and receive highly complex, highly sensitive and highly contentious information, where there may be substantial barriers to acceptance, using enhanced persuasive and empathetic communication and reassurance skills as required.
* Ensure continual quality monitoring and improvement focusing on reducing risk and developing efficiency and quality in care.
