Therapy Healthcare Support Worker (Band 3) 1.0 wte (37.5 hours) over 5 days / Permanent / Rotherham Stroke Rehabilitation Team. We have a fantastic opportunity for a Therapy Healthcare Support Worker to join our Stroke Rehabilitation Team. Our multidisciplinary team includes SLTs, Occupational Therapists, Physiotherapists, Clinical Psychologists, Assistant Practitioners, Therapy Support and admin staff, enabling holistic and bespoke rehabilitation. We provide a pathway of integrated care across the Stroke Recovery Unit (acute and rehab) and the Integrated Community Stroke Service (early supported discharge and community stroke rehab up to 6 months). We are the only therapy team across South Yorkshire and Bassetlaw to work in this innovative and dynamic way, with seamless transfer of care for stroke survivors and their careers. We always put the stroke survivor, their careers and the people we work with at the centre of every decision we make.
Main duties of the job
Working alongside a large multi-disciplinary team, we will support you to provide a high quality, efficient service to maximise people’s independence and function with the aim of facilitating and managing a safe and timely hospital discharge and ongoing rehabilitation. You will contribute to national targets associated with this specialty (SSNAP). CPD is encouraged and supported. Supervision within the role and opportunities for clinical and leadership skills development will be provided. Rehabilitation experience is desirable but not essential as appropriate training will be provided.
Responsibilities
* Physically capable of assisting patients with exercises, mobility and daily living activities.
* Travel independently across Rotherham and have access to a suitable vehicle for business purposes (adjustments considered under Equality Act 2010 where necessary).
* Support a high quality, patient-centred rehabilitation service in collaboration with the multidisciplinary team.
Working pattern and recruitment
The post will be 5 days, Monday to Friday, with a contribution to weekend and bank holiday rota. Applications from recently qualified registered Allied Health Professionals are welcome. Person Specification
Experience
* Essential: Experience of care; recent and relevant NHS or equivalent experience; experience of working within a multi-professional team.
* Desirable: Experience of exercise/rehab roles.
Knowledge and Awareness
* Essential: Understanding of the role of the support worker in rehabilitation/management of patients in the specialty; understanding of confidentiality; knowledge and understanding of health and safety; ability to recognise when guidance and support is required.
Qualifications
* Essential: NVQ level 3 in a relevant field or equivalent or willingness to work towards; Maths & English at Functional Skills level 2 (or GCSE or equivalent).
* Desirable: Care Certificate.

* Essential: Ability to travel independently between TRFT sites and for patient visits in the Rotherham area; access to a suitable vehicle for business use; effective communication; ability to convey sensitive information; proficient with computer systems; responsiveness in urgent/emergency situations; data collection to inform service decisions; team player; approachable; resilient; flexible; genuine care for people.
