Socialprescribers help to reduce health inequalities by supporting people to unpickcomplex issues affecting their wellbeing and enabling them to have more controlover their lives. Through creating personalised health and wellbeing plans,goals are set to help people overcome barriers they face, by connecting themwith local VCSE organisations, community, and neighbourhood groups.
Ideallyyou will have a good knowledge of health and social care and voluntary servicesin the area and be keen to take on a new challenge as a Social Prescribing LinkWorker within our successful team. You will be person-centred, dedicated andcommitted to making a positive difference to the lives of our patients. Workingin a supportive patient facing role, you will build strong relationships withthe voluntary, community and social enterprise sector. As our SocialPrescribing Link Worker, you would also work with a wider team of socialprescribers throughout Trafford and Greater Manchester and be integral tofuture plans of the VCSE sector and health and social care in the area.
Main duties of the job
The role includes, but is not limited to:
Promote social prescribing, its role in self-management,addressing health inequalities and the wider determinants of health.
As part of the PCN multi-disciplinary team, build relationshipswith staff in GP practices within the local PCN and Trafford PCNs, attendingrelevant MDT meetings, providing information and communicating feedback onsocial prescribing.
Meet people on a one-to-one basis. Give people time totell their stories and focus on 'what matters to me'.
Help people identify the wider issues that impact on their healthand wellbeing, such as debt, poor housing, being unemployed, loneliness andcaring responsibilities.
Work with individuals to co-produce a simple personalised supportplan to address the persons health and wellbeing needs based on the personspriorities, interests, values, cultural and religious/faith needs andmotivations.
Where appropriate, physically introduce people to culturallyappropriate community groups, activities and statutory services, ensuring theyare comfortable, feel valued and respected.
Forge strong links with a wide range of local VCSE organisations,community and neighbourhood level groups, utilising their networks and buildingon whats already available to create a menu of diverse community groups andassets, who promote diversity and inclusion.

Altrincham Healthcare Alliance PCN is aforward-thinking Primary Care Network consisting of five GP practices in southTrafford. We are committed to providing patient-centered healthcare by working togetherwith community, mental health, social care and voluntary services in our area. You will work alongside GPs, Nurses, Physios, Pharmacists and other practice staff at each GP practice to support patients with Social Prescribing.
All staff have access to the NHS pension scheme.
