Wiltshire Health and Care are activelysupporting this through acute admission avoidance, urgent crisis response, 24hour community care, and enhanced care for patients living with long termconditions. As more and more patients are treated outside of a traditionalhealthcare setting, the demand for community nurses in the UK is growingrapidly to ensure a high standard of care to everyone who needs it.
Becomingacommunity nurseenables you to trulygive back to your community on the front line of healthcare. Often workingwithout the support and resources of a hospital environment, you will need tobe able to adapt well and quickly to new environments: this job is perfect forsomeone who wants to challenge themselves and develop their skillset, whilstgiving back to the community.
Alongside your clinical role in your patients lives, you will alsouniquely serve as a teacher and educator to the patient. You will be informingthem of future ways in which they can supplement and administer their own careor instruct their family on how best to care for them. This will ensure thatthe patient is receiving the best possible treatment in any scenario.
Main duties of the job
We have an opportunity within Warminster Westbury Community Team to employa Band 5 Nurse. Our nurses are expected to work seamlessly within themulti-professional community teams, working closely with primary and secondarycare, and members of the voluntary sector We are integrated co-locatedmulti-disciplinary teams which create a wrap around approach in the delivery ofexcellent patient care to our diverse population. Within our team we haveDistrict Nurses, Physiotherapists, Occupational Therapists, Care Co-ordinators,Health Care Support Workers, and Home First Rehabilitation Support Workers.
We actively encourage development of all our staff,offering regular in-service training and professional development including thechance to become a Nurse with Special Interest.
We value staff members who embrace change and arekeen to help develop and shape future services. The successful candidate willbe able to provide clear evidence of up to date, relevant clinical practice andeducation. Applicants need to have well developed communication skills, good ITknowledge, and evidence of basic clinical skills. We are looking for anenthusiastic, motivated and hardworking individual who can work flexibly andefficiently. The team operates various shift patterns from until, 7days a week.
