This is an opportunity to join the team as a secondment for 6 months to support Critical Care Outreach in the roll out of Martha's Rule within the trust.

The post holder is responsible for clinical leadership through effective monitoring and implementation of standards and evidenced based practice; ensuring patients receive safe, high quality care.

To provide a high standard of care supporting the critical care outreach team.

To provide direct management and facilitate learning and development within the nursing team.

As a member of the outreach team, provide expert clinical skills and support to the wards and departments as part of the multi-disciplinary team.

To help roll out Martha's Rule with the trust

Support Critical Care Outreach team in responding to Martha's Rule calls

Take responsibility for the clinical leadership and direction of the critical care team

Undertakes and documents a comprehensive, systematic and accurate nursing assessment of the patient as appropriate

Deliver a high standard of care based on best practice to maintain patient safety and provide a good patient experience.

Creates and utilizes opportunities to promote health and well-being of patients in their care

To undertake clinical shifts within the outreach service, identify, assess and manage a caseload of patients who are deteriorating or who are at risk of deteriorating. Recommend interventions to optimise patient's care, obtaining medical support as required. Support staff that are caring for patients awaiting transfer to a critical care bed.

Review patients at ward level and respond to requests for support from nursing and medical staff.
