It is an exciting time to join our expanding rapid response and intermediate care teams within a Support worker/HCA role. This role involves working as part of a multidisciplinary team with Nurses, Occupational Therapists, and Physiotherapists.
The team aims to support hospital discharges, prevent admissions, and help people remain as independent as possible at home through rehabilitation programmes.
Our base office is at Sheppey Community Hospital; however, you will be provided with equipment to enable mobile working, and virtual platforms are used for handovers to maximize productivity.
Main duties of the job
* Work actively as a member of the multidisciplinary team (MDT) to deliver high-quality nursing, occupational therapy, and physiotherapy interventions.
* Participate in patients' individual rehabilitation and care programmes under professional guidance.
* Collaborate with the MDT, GPs, and local hospitals to provide rapid response short-term interventions via the Discharge to Assess (D2A) and 2-hour rapid response pathways.
* Complete basic assessments under supervision and undertake Trusted Assessor Training when available.
* Promote high standards of patient rehabilitation.
* Work autonomously and as part of a team.
* Seek additional tasks to support the team, such as administrative duties, being the first point of contact, and maintaining service equipment.
