Community Cardiovascular Staff Nurse

This fixedterm secondment opportunity offers an exciting opportunity for a suitablyqualified and experienced Nurse to work within the Specialist Coronary HeartDisease & Heart Failure Team based at the Whitehouse in Easington. The postcovers core hours of 09:00-17:00 Monday-Friday with currently no weekendworking although that may change in the future


Main duties of the job

The successful applicant will be primarily caring forpatients with coronary heart disease (CHD) referred into the service followingacute events eg myocardial infarction and planned procedures eg elective PCI.They will also be expected to support the Heart Failure Specialist Nursesreviewing and caring for heart failure (HF) patients.

They will be directly involved in delivering cardiacrehabilitation, to both CHD and HF patients, alongside the exercisefacilitators.

There may be an opportunity for professional development,for example, attend courses relevant to CHD/HF.

Care is delivered in the patients own home andin clinic / community venues around Easington

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities

The jobs entails offering nursing care, support andmonitoring to patients with cardiovascular disease and heart failure

It involves delivering cardiac rehabilitation both face toface in classes and home based (heart manuals and/or digital).

It involves support to optimise treatments prescribedfollowing diagnosis and monitoring effects.

It involves education and support around lifestyle issuesand risk reduction

The job entails independent travel around theEasington/Seaham area with possible need to support other teams across theCDDFT footprint..


Person Specification


Qualifications

* Registered Nurse.
* Professional knowledge acquired through degree or equivalent level of demonstrable professional knowledge/experience.
* Knowledge / experience of cardiac rehabilitation


Experience

* Up to date knowledge of professional practice.
* Knowledge of national and regional policies influencing cardiovascular disease
* Evidence of team working.
* Post registration experience of working with cardiac patients.
* Experience of working in a community setting.
* Experience of audit and/or service development/review.


Statutory Registration

* Registered health professional registered with the NMC


Special Skills & Knowledge

* Excellent communication and interpersonal skills.
* Good organisational and problem solving skills.
* Understand cardiovascular anatomy and function.
* Understand common cardiovascular pathophysiology and cardiac conditions.
* Knowledge of national and regional policies influencing cardiovascular disease.
* Ability to undertake holistic cardiovascular assessment and review or willingness to work towards.
* Good organisational and problem solving skills.
* Accurately record clinical data and understand their significance: BP, heart rate and rhythm, pulse, oximetry, and respiratory rate.
* Phlebotomy or willing to work towards.
* Evidence of team working and care planning.
* Ability to work independently and with minimal supervision.
* Ability to act alone in an emergency situation.
* Able to prioritise work on a daily basis to meet service demands.
* Confident in the use of email and basic IT skills.
* Understand use of health related questionnaires and patient reported outcome measures.
* Participates in and provides clinical supervision.
* Understand benefits of non-pharmacological interventions for cardiovascular illnesses/conditions such as cardiac rehabilitation, smoking cessation interventions, healthy eating/weight management and exercise.
* Knowledge of self-management. Able to travel independently around County Durham & Darlington.
* Flexible attitude to work.
* Able to safely move and handle loads
