This is an exciting opportunity to be part of cutting‑edge healthcare innovation that will impact thousands of patients and save lives. Hampshire Hospitals NHS Foundation Trust (HHFT) has received funding for further development of the Virtual Health Hub, including the virtual wards following the success of COVID virtual wards. The role will support the managing and embedding of lessons learned through the COVID‑19 pandemic across the region by upscaling Hampshire virtual care regionally to deliver sustainable transformation at ICP level.


Main duties of the job

* Use change and project management skills to support the management of senior external stakeholders and HHFT stakeholders, including close collaboration between primary and secondary care to deliver the project.
* Contribute to the governance of the Virtual Health Hub through participation in research, audit and user feedback, ensuring that discussions and decisions are informed by up‑to‑date clinical evidence.
* Support the delivery of digital transformation that will improve outcomes for residents, link in with other technology initiatives, and provide support to other projects and programmes of work within the team.
* Work as an autonomous practitioner with a caseload of patients within the Virtual Health Hub team structure, providing rapid assessments to patients referred from system‑wide partner organisations such as GP’s, Ambulance Services, and Care Homes.

Training and Qualifications


Essential criteria

* Professional UK registration e.g., NMC, HPC, GPhC
* Formal training &/or experience in mentorship
* Management and Leadership Development


Desirable criteria

* Additional postgraduate learning in specialist field including master's degree or working towards
* Project management learning and/or development


Experience and Knowledge


Essential criteria

* Evidence of Continued Professional Development (CPD) clearly recorded for professional profile
* Evidence of post‑registration clinical work experience in the speciality
* Understanding of Health Environment policies and legislation including Clinical, HR, E&D, Governance


Desirable criteria

* Experience of change management
* Experience of project management for policy development or large‑scale projects
* Experience of remote video assessment or telephone triage service


Skills and Ability


Essential criteria

* IT‑literate
* Understanding of effective clinical governance including implications, quality, and audit
* Advanced communication skills including to communicate complex, sensitive, or confidential information in an appropriate manner; to liaise effectively; to understand and disseminate multifaceted information.
* UK driving licence


Desirable criteria

* Highly specialised clinical skills across the relevant speciality
* The ability to work at problems from a unique or different angle
* Proven remote video assessment skills
