JobDescription:
* Takeresponsibility for the management of a complex caseload whichincludes specialist assessment (within service specification) careplanning in conjunction with service user and carer evaluation ofindividual treatment plans for CYP with learningdisabilities.
* Responsible for participating inthe setting of quality standards including the auditing monitoringand reviewing of practice in line with current clinical guidancepractice and policy.
* Assess patients individualcare needs develop implement and evaluate programmes ofcare.
* To work as part of the ChildrensCommunity Learning Disability Team assessing the health andemotional wellbeing needs of children and young people withmoderate to severe learning disabilities and respondingappropriately.
* To complete specialist nursingassessment development implementation and evaluation ofinterventions to promote the health and wellbeing of children youngpeople and families affected by learning disabilities and orcomplex physical and mental health needs.
* Towork as an integral member of the multi disability team maintainingresponsibility for a designated caseload acting with autonomy asappropriate.
* To positively represent the needsand rights of children and young people with learning disabilitiespromoting equality and access to NHS and any otherservice.
