Responsibilities
* Support the ACERS Heart Failure and Rapid Diuresis Response/Virtual Ward service with the delivery of care and administrative processes.
* Practice within a multidisciplinary team and provide care to patients within their own home and community settings.
* Implement and monitor care needs under the supervision of registered nurses.
* Take responsibility for own actions when delivering delegated clinical care, maintaining competency and skill in line with role requirements.
* Provide excellent communication with patients, relatives and other MDT members.
* Work closely with the team clinicians to efficiently process new referrals, enquiries, and collate information.
* Maintain effective administrative processes for the ACERS Heart Failure service.
* Be required to work across multiple sites.
* Undertake delegated clinical/technical tasks following competency assessment (e.g., phlebotomy, ECG recording).
* Undertake delegated clinical observations including vital signs, blood sugar, and skin integrity assessment.
* Identify changes in patients’ condition and, where appropriate, recognise when significant changes in the care plan are required and report back to senior clinicians promptly.
Qualifications
* Essential Education: GCSE maths/English or equivalent level of education; Basic numeracy and literacy skills.
* Desirable Education: Care Certificate.
* Essential Skills/Abilties: Venepuncture trained and proficient; clear verbal communication; able to report accurately both verbally and in writing.
* Desirable Skills: Trained and proficient in recording ECGs.
* Essential Values/Experience: Understanding of Trust Values; experience of working in a clinic/outpatient environment.
* Desirable Experience: Experience of working in the Community.
