Medical Nurse Practitioner – Older People

Are you looking for a new challenge as a Medical Nurse Practitioner making a real difference for older people living with frailty? We are recruiting for an enthusiastic and motivated B6/7 MNP post to work within the supportive and forward‑thinking team at the RUH.
You will work closely with consultants, ACPs, nursing and therapy colleagues in ED and the front door, and core wards delivering multi‑disciplinary, patient‑centred care, advising on discharge or admission with the complex caseload this interesting group provides. You will be supported with teaching and development skills and opportunities to build your own knowledge and learning and shape your future pathways in this area.
Responsibilities
* Hold responsibility for own case load and provide a high standard of assessment, diagnosis and planning for:
o Frailty – early assessment and intervention in the emergency department and Older Person’s Rapid Assessment Area (OPRAA) together with geriatricians, medical nurse practitioners, occupational therapists and physiotherapists.
o The Older Person’s Assessment Unit (OPAU) and Older Person’s Unit Short Stay (OPUSS) – short‑stay inpatient ward for patients requiring a bed for less than 72 hours.
o Core wards – Combe and Waterhouse.
* Provide clinical support, teaching and education to colleagues and families.
* Support with future planning and management of frail patients.
* Represent MNPs and work with other members of the MDT to support service improvement and best practice.
