Social Work Support Assistant, Adult Social Care - Permanent, Full-time (37 hours), South Care and Support Team based in Blyth.
We have an exciting opportunity for a Social Work Support Assistant / Care Manager 1 to join our South Care and Support Team.
We are looking for a passionate person, keen to deliver high-quality service, to join our team.
About the role:
As a key worker for adults over 18 with care needs, you will be the main contact for support and advice to them and their families. You will ensure their safety and help them live as independently as possible.
The role includes completing needs assessments, considering safeguarding, reviewing healthcare needs, and promoting self-directed support.
You will work within a Care and Support Team, including social workers and care managers, who coordinate with GP practices, community health services, and district nurses.
You must be committed to enabling service users to live independently and think creatively about solutions, with a focus on risk management and protection from harm.
You should work well independently and in a team, with good time management, prioritization skills, and strong interpersonal, advocacy, and communication skills. Solid IT skills are required for data input and retrieval, with full system training provided.
Main responsibilities include:
1. Carrying out assessments and designing solutions with service users to meet their outcomes, seeking guidance when needed.
2. Managing and planning work effectively for monitoring and reviewing outcomes.
3. Undertaking Carers' Assessments as part of care management.
4. Carrying out Risk Assessments in care management.
5. Participating in the Duty System for ad hoc work, processing information, and acting under supervision.
6. Shadowing students and new staff from other disciplines.
7. Participating in discharge and care planning meetings.
8. Maintaining written and electronic records to organizational standards.
9. Providing reports, assessment outcomes, and recommendations.
10. Advocating on behalf of service users.
Essential requirements:
* NVQ 3 in social care or equivalent
* Experience working with adults in social care
* Knowledge of legislation relevant to mental health and adult social care
* Experience working with individuals with mental illness, learning disabilities, or physical conditions in adults and older people
* A valid driving license and access to a car for work purposes
