A fantastic opportunity has arisen within the Adult Community Learning Disability team for a Band 7 Specialist Nurse, Community Practitioner to join our team.
A team made up of a wide range of MDT principles with a friendly specialist approach to supporting individuals with a learning disability maintain independence within the community. Overcoming health inequalities and promoting wellbeing our team has the fortunate opportunity to enhance the lives of our patients.
The Specialist Nurse will provide a high level of nursing care and treatment to Service Users accessing the service with high care needs due to their complex difficulties and/or associated risks.
To deliver a high quality of nursing care, assessment and therapeutic intervention to adults with complex and substantial difficulties as part of a multidisciplinary team approach.
To manage complex risks competently and in accordance with Trust and Statutory guidance.
The post holder will facilitate access to mainstream primary care services for adults with mental health and/or learning disabilities and / cognitive problems and to advise and support agencies with regard to meeting their duty of care.
The post holder will directly influence, inform and advise on clinical training needs and provide specialist support, supervision and training.
The post holder will be responsible for identifying health inequalities give advice as to reasonable adjustments that can be made in order to ensure services are accessible.
Support the team managers with team development and other practitioner performance and if applicable to the service, assist the clinical managers and contribute to individual practitioners' performance in relation to non-medical prescribing.
Participate in the planning, development and evaluation of clinical services leading on defined projects as agreed with the Clinical Manager and in collaboration with the Clinical Lead and Team Manager.
