William Hopwood Surgery are currently looking for a full-time Quality & Improvement Lead to join our dynamic and evolving organisation. Previous experience of working in a busy general practice environment is preferred.

Main duties of the job
The successful candidate will be required to work as part of a team with opportunity to bring in their own initiatives and ideas
5. To work with the Practice Leadership team to identify, implement and review the systems within the Practice, to improve patient outcomes and maximise quality outcome points.
6. To utilise Practice and patient data to improve performance and assist the Practice in meeting targets and improving the provision of health to the Practice population.
Main duties of the job
Promote national, local and practice quality standards forchronic disease management, to ensure that the quality of care is maintained ata high standard using NICE guidance and the existing GP contract as a guide.
Work with senior leadership team to develop strategies forimproving patient outcomes and maximising income for the Practice using QoF andenhanced services both local and national like IIF,QI,QOEST.
Utilise available data and quality improvement strategy toidentify patients requiring long term condition and QoF reviews and ensure thisprocess runs efficiently.

Job responsibilities
Utilise available data and quality improvement strategy and ensure thisprocess runs efficiently by :
7. Running monthly reports for individual QOF domains andcontact the patients using SMS, telephone, email or letter to ensure contact ismade within the correct time scales and the correct read codes are used forreporting purposes
8. Running a monthly report for all enhanced services,including high risk drug monitoring recalling patients as above.
9. Running a monthly report and inviting patients in fortheir NHS health check.
10. Running monthly CQC searches, completing any actionrequired and updating the monitoring spreadsheet, highlighting any issues tothe senior management team.
11. Supporting the Practice manager in the submission of monthend/year end data.
12. Organising and recalling of eligible patients forvaccination clinics/home visits.
13. Assist and undertake clinical andadministrative audits
14. Check and correct patient coding against business rulesfor GP contracts QOF/ QI/ IIF/ QOEST
15. Work with clinical and non-clinical staff to improve theaccuracy of data input
16. Assist with maintaining accurate disease and QoF registerswithin the clinical system
17. Proactively seek and implement a variety of quality and improvement projects, including those directed by the senior leadership team including review significant events and complaints processes inline with Practice policy.

Qualifications
Essential
18. GCSE Level 6 in English and Maths
19. An understanding of EMIS, including running clinical searches
20. Sound understanding of quality and improvement tools like QOf / IIF
21. Good communication and organisation skills
22. Experience of working in a team ideally GP Practice
23. Good analytical skills
24. Good IT skills
25. Ability to work under pressure and to deadlines
26. Ability to prioritise and track own workload
