Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. In this pivotal role, you will be responsible for ensuring that our clients receive the highest quality of care and support by managers the rotas effectively. You will act as a liaison between clients, families, and healthcare providers, facilitating communication and coordinating services to meet individual needs. The ideal candidate will possess strong organisational skills and have previous experience in managing rotas for domiciliary care workers. You will need to be able to plan ahead, deal with, and be ready for problem solving. A good knowledge of the Wirral is a requirement of the role and the ability to work well under pressure is advantageous.
Responsibilities
* Coordinate carer rotas based on individual needs and preferences.
* Maintain accurate records of client interactions, carer interactions, and progress notes in compliance with confidentiality regulations.
* Provide information and resources to clients and their families regarding available services and support options.
* Monitor carer progress and adjust rotas as needed in response to changing circumstances or needs.
* Participate in team meetings to discuss client/carer cases and share insights for improving care delivery.
Qualifications
* Previous office experience is essential; familiarity with healthcare settings is advantageous.
* Strong communication skills, both verbal and written, with the ability to convey information clearly and empathetically.
* Excellent organisational skills with a keen attention to detail.
* Ability to work independently as well as part of a multidisciplinary team.
* Proficient in using computer systems for record keeping and communication purposes.
* A compassionate approach towards carers, clients and their families, demonstrating patience and understanding in all interactions.
