An exiting opportunity to help succession plan for the future of IBDS services. With the additional activity and forward planning to future proof our workforce and accommodate the increased number of patients in our care. The post holder will assist in the delivery of high quality Inflammatory Bowel Disease (IBD) services within the trust.

Working alongside other clinical colleagues to provide expert care, treatment and support for this patient group. The post holder will contribute to advancing the body of knowledge through education, research and audit as well as developing practice. They will play a key role within the multidisciplinary team in order to ensure a seamless, coordinated and timely approach to care.


Main duties of the job

work alongside the IBD ACP and support nurse in providing a nursing service for IBD patients.

* To provide nurse led follow up clinics working autonomously in providing care and education to IBD patients and their families/ carers.
* To provide specialist advice via the telephone advice line/ HOT IBD using telephone or face to face clinics.
* To provide advice to both patients and staff regarding the management of IBD.
* To promote patient own self-management and help address psychological issues where appropriate.
* To encourage independence and empower patients and carers.
* To promote compliance with treatment plans.
* To actively educate patients, carers and staff in the management of IBD
* To comply plans of care for patients monitoring response to treatment and manage accordingly.
* Assess and triage patients via the advice/ help line.
* To collaborate across the multi-disciplinary team, with patients and primary workers.
* To work autonomously and be accountable for own professional actions.

Job responsibilities

The post holder will assist in the delivery of high quality Inflammatory Bowel Disease (IBD) services within the trust. Working alongside other clinical colleagues to provide expert care, treatment and support for this patient group. The post holder will contribute to advancing the body of knowledge through education, research and audit as well as developing practice. They will play a key role within the multidisciplinary team in order to ensure a seamless, coordinated and timely approach to care.Actively participate in audit work.

* Ensure clinical practice reflects current research.
* Assist the IBD support nurse with the supervision of patients receiving specialised treatments such as biologics.
* Assist with coordination of pre biological investigations and checks as well as those on immunosuppressant therapy according to agreed protocols.
* Liaise with ward staff in providing support for patients admitted with exacerbation of IBD or newly diagnosed patients.
* Ability to communicate effectively at ALL levels.
* Dealing effectively with distressing emotional episodes with patients following consultant diagnosis

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.


Person Specification


Education & Qualifications

* Professional Registration
* Higher Education qualification in speciality field
* Non-medical prescribing


Knowledge

* Evidence of implementing change
* Evidence of research and audit
* Evidence of dissemination of knowledge in wider healthcare organisations
* Evidence of managing patients with IBD
* Advanced clinical practice skills


Experience

* Post registration experience at a senior level (band 6 or above) in a speciality area
* Evidence of teaching in a variety of environments
* Experience of using multidisciplinary approach to patient care


Skills & Abilities

* Advanced clinical practice skills
* Strong leadership skills
* Excellent communication skills and able to communicate with MDT
* Sound clinical judgment in identifying and ordering diagnostic tests / interventions
* Able to work as part of a team and autonomously
* Clinical assessment of healthcare needs
