An exciting opportunity has arisen for a compassionate and motivated Community Mental Health Support Worker to join the Sutherland Integrated Health and Social Care Team, based at Assynt Medical Practice in Lochinver. This rewarding role involves supporting individuals experiencing mental health difficulties within their own homes and local communities across West Sutherland. You will work closely with Community Mental Health Nurses and the wider multidisciplinary team to deliver person-centred, recovery-focused care, helping individuals to maintain independence, improve wellbeing and engage with services.
Key responsibilities
* Provide practical and emotional support to individuals, assisting with care plans and promoting self‑management.
* Support early intervention to prevent deterioration and reduce the need for more intensive services.
* Work independently and as part of a team within a community setting.
Qualifications and skills
* Strong communication skills, empathy, and the ability to work both independently and as part of a team.
* Full driving licence; access to a vehicle for work purposes may be required.
