We currently have an exciting opportunity fora talentedhealth and wellbeing co-ordinator, to joinourestablished teaminEdinburgh.Asweembark on the next stage ofourjourney deliveringtrulyoutstanding, highly personalisedcare and support services.
Ourspecialistoutreach team provides supportto adults within their own home. Individuals we support can have a range ofdiagnoses, including mental health conditionsand/or learning disabilities. Some may also experience addiction and have a history of offending behaviour. The team however focus on the individual’s abilities.

Who are we looking for?
The ideal candidate will:
Haveexperience of workingwithin health and social care and demonstrate aninnovativeand empatheticapproach todelivering and leading services.
Have a passion and commitment to delivering person-centred support.
Have experience of working as part of a staff team, or lone working and experience of co-production of support, to enable individuals to meet and achieve their personal outcomes.
Be able to assist in delivering an outstanding service and have experience or knowledge of the effects homelessness can bring to individuals.
