Main duties
This advert is for vacancies in the NORTH LOCALITY TEAM. The post holder will work as Care Coordinator / Community Mental Health Nurse for a caseload of service users in the local community with complex mental health needs, and with their carers where applicable. The role requires excellent self-management, collaboration within the team, ability to formulate holistic care plans with service users and/or carers, ability to work under pressure, dynamic risk assessment, and excellent communication skills. Vaccination against Covid-19 evidence may be required in line with regulatory obligations for care home visits.

Qualifications
* Relevant professional qualification in Mental Health Nursing, Social Work, or Occupational Therapy with affiliated professional registration (NMC, HCPC).
Experience
* Experience of interagency working
* Experience of managing a caseload
* Significant relevant clinical experience
* Experience of working with diverse groups of patients and staff
* Experience of working in the community
* Experience of supporting practice development in clinical areas
Skills and Abilities
* Knowledge of CPA, Community Care Act, Carers Act, Mental Health Act and related legislation
* Skills in mental health and risk assessments
* Understanding of Care Coordination and CPA
* Ability to respond to service users’ and carers’ needs
* Understanding of recovery in mental health
* Strong care planning and holistic care approach
* Problem solving and decision-making abilities
* Experience in supervision and giving constructive feedback
* Knowledge of clinical governance and audit

* Empathy for service users, including those with mental health problems
* Ability to fulfil all role requirements (including physical)
