Job Overview

Carlisle Healthcare District Nursing Team are an emerging and unique team that put people at the heart of what we do. We are looking to appoint two motivated and enthusiastic Senior Community Staff Nurses to work as part of our dedicated Community Nursing team and the wider ICC. This role involves the delivery of holistic, patient-centred care in the community as well as complex caseload management. The successful applicant will liaise with the primary health care team and other Health and Social Care professionals in care provision and work across Carlisle and the wider Community nursing service across North Cumbria.


Main Duties of the Job

You must be able to demonstrate the ability to work within a changing environment and have good communication skills. The successful applicant must be able to work independently and as part of a team. You will be supported by the District Nursing Team Lead and Band 6 District/Community Nurses to deliver nursing care covering weekdays, weekends, bank holidays and evenings.

This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, support patients with complex needs within the community setting, and support the Team Lead to provide clinical leadership, operational management of the community nurse team and management of the clinical caseload for the team. Community experience is therefore desirable.

Person Specification


Qualifications

* Level 1 registered nursing qualification
* Evidence of continuous professional development to degree level or equivalent experience
* Post Registration qualification in Acute and Chronic disease management
* Nurse Prescribing
* Specialist qualification in Community Nursing or equivalent


Experience

* Ability to use initiative, prioritise effectively.
* Demonstrate evidence to work as part of a team
* Demonstrate evidence to motivate team/peers and manage change
* Problem solving skills and critical decision making
* Leadership
* Operational clinical management of a team
* Knowledge of changing health agenda and impact for community nursing
* Demonstrate recent community experience


Knowledge

* Demonstrable understanding of the extent of the role.
* Broad knowledge base of acute and chronic conditions.
* Understanding of scope of professional practice.
* Understanding of reflective practice and its application.
* Self awareness and appreciation of limitations.
* Knowledge of changing way health services are being planned and delivered
* Awareness of current changes in the delivery of healthcare and the impact on community services


Skills and Aptitudes

* Excellent communication and interpersonal skills.
* Ability to maintain confidentiality.
* Enthusiasm and ability to work as part of a team.
* Post registration clinical skills


Personal Circumstances

* Able to travel independently
* Able to work 24/7 and flexibly across a range of in-patient and community settings.
