Cancer Diagnostic Facilitator
Contract: 9 month fixed term
Hours: 37.5 hours per week
To assist the Cancer Services Team in managing patient pathways, ensuring patients are reviewed, diagnosed, and treated within the National Cancer Waiting Time targets.
Assist in processes that provide Quality Assurance of cancer services provided by the Trust. Collect, record, and validate a wide range of cancer data required for national cancer data sets.
This includes monitoring referrals, test requests, reports, and actions taken. Contact patients, gather data, and provide real-time feedback to internal and external teams.
Coordinate with various departments and staff to gather information for cancer MDT meetings, ensuring timely availability of information for clinical decision-making.
Support MDT Coordinators and Pathway Navigators in managing patient pathways.
Contact GP practices and patients regarding at-home testing to facilitate clinical triage in a timely manner. Liaise with departments to obtain information necessary for clinical diagnosis and prompt diagnostic appointments.
