About the Team and Role
The Social Care Connect Team is the first point of contact for Adult Social Care, supporting people aged 18 and over. We are recruiting two full-time Social Care Officers to join this fast-paced and demanding service.
The role is varied and dynamic, requiring flexibility, sound judgement and the ability to manage competing priorities. You will work with adults whose needs may be short-term or time-limited, as well as those who require further assessment or support planning.
You will play a key role in gathering information, completing brief forms and/or full Care Act assessments, applying proportionate decision-making, and supporting timely outcomes that promote independence and help prevent unnecessary escalation of needs.
While duties may vary (in line with the job description and organisational needs), the role requires confidence in working with people following changes in their circumstances, responding to fluctuating needs, and contributing to effective, outcome-focused adult social care pathways.
What You Will Do:
1. Act as a first point of contact for adult social care enquiries
2. Engage with individuals and professionals to understand current needs and circumstances
3. Provide clear advice, information and signposting to appropriate services
4. Support timely, strengths-based decision-making and information gathering
5. Contribute to proportionate triaging and short-term intervention planning
6. Complete Care Act assessments and support planning, for adult who are eligible for support.
7. Work calmly and professionally within a fast-paced, high-volume environment
