Job Title & Summary
We’re seeking an experienced clinical pharmacist to join our Elderly Care and Frailty Team at the University Hospital of North Midlands NHS Trust, based at Royal Stoke University Hospital. The role involves delivering high‑quality clinical pharmacy services, supporting the Frailty Emergency Assessment Unit and Frailty Same‑Day Emergency Care, and developing the clinical service.
Responsibilities
* Manage elderly and frail patients using expert pharmaceutical knowledge.
* Practice independent prescribing (or work to obtain qualification) within scope of practice.
* Provide prescriber support for complex medicine regimes (e.g., gentamicin, vancomycin, aminophylline infusions).
* Assist efficient discharge medication supply routes in and out of hours.
* Review adverse incident reports and educate medical & nursing teams on pharmaceutical issues.
* Develop guidelines and PGDs, and cost‑improvement initiatives.
* Train, develop and line‑manage junior and trainee pharmacists.
* Collaborate with Advanced Clinical Lead Pharmacist for Medicine on strategic development of the service.
Essential Qualifications
* Master’s degree in pharmacy (MPharm) or equivalent.
* Registered with the General Pharmaceutical Council (GPhC).
* Postgraduate Clinical Pharmacy Diploma / MSc or equivalent qualification.
* Independent Prescriber annotation on GPhC Register (or willingness to undertake).
* Extensive hospital pharmacy experience.
* Comprehensive clinical pharmacist practitioner experience across clinical specialties.
* Education and training experience, including supervision and appraisal of junior staff and students.
Desirable Qualifications
* Formal management and/or leadership qualification.
* Recent clinical pharmacy experience in Elderly Care and Frailty.
* Evidence of involvement in pharmacy practice research – publications or conference presentations.
* Experience managing a team and undertaking appraisals.
Experience Requirements
* Extensive hospital pharmacy experience.
* Comprehensive clinical pharmacist practitioner experience across several specialties.
* Supervision, appraisal and training of junior pharmacists and students.
* Recent experience in Elderly Care and Frailty (desirable).
Salary, Contract & Working Pattern
Band 8a – £57,528 to £64,750 per annum. Fixed‑term contract for 9 months, full‑time. Working pattern: Full‑time. Reference number: 205‑7982662.
