Community Integrated Care is currently seeking a dedicated and compassionate health and social care professional for a Service Leader Level 2 position based at Crawford Gardens, Huyton, Liverpool, L36.
Crawford Gardens is an Extra Care Scheme with 110 residents.
Extra Care housing is designed for people who are predominantly over the age of 55 years and offers the benefit of independent living with on‑site support available when needed.
CQC Registration will be required for the successful applicant.

Responsibilities
* Lead an engaged and motivated team to change lives and deliver person‑centred outcomes for every person we support.
* Drive performance, set pace and direction within services, promote a positive and passionate culture.
* Ensure services are appropriately and effectively resourced to change lives for the better.
* Experience working in a health or social care setting; maintain an upbeat positive manner and genuine passion for supporting people to become part of their local and wider community.
* Experience working with people supported who present behaviours that require intensive support planning and management to ensure safety and quality of life at all times.
* Comfortable using basic IT systems required within the role; training can be offered to support this area.
Qualifications and Skills
* Professional qualification in Health and Social Care (e.g., Nurse, Social Worker, SVQ Level 3 plus supervisory units (Scotland) or Care Management Level 3 (England); preferably Level 5).
* Relevant experience in health or social care supporting people requiring intensive support planning and management.
* Strong leadership skills and a strong work ethic.
* Patience, empathy, compassion and sensitivity to others; reliability, honesty, determination, problem‑solving, resilience.
* Ability to work within and promote a diverse, inclusive, and respectful workplace culture.
