Discharge Liaison Officer
The Discharge Liaison Officer is a new and exciting role supporting and facilitating the safe and timely discharge of patients.
The key element of the role will be to communicate with BTHFT colleagues, the multidisciplinary team, patient, families, medical and nursing staff to ensure everyone is working to the same discharge plan. This list is not exhaustive and other key relationships are expected to develop within this role.
Responsibilities
* Prepare to work 7.5‑hour shifts, five days per week.
* Support the MAIDT to facilitate effective discharge.
