Our service is a busy, supportive environment made up of three integrated teams: Home First, the Integrated Discharge Hub, and the Community Bed Team. We are co‑located to provide a strong multidisciplinary approach, ensuring the best possible outcomes for the people we support.
Main duties of the job
* Undertake person‑centred wellbeing assessments
* Work collaboratively with health professionals and partner agencies
* Support timely hospital discharges and reduce delays
* Manage a varied caseload within a 7‑day service
Job responsibilities
Ensure that people are fully involved and empowered throughout their contact with the service. Ensure that the person is fully understood in order that the most appropriate tailored solutions can be offered. Identify and address any system barriers which prevent the people from experiencing the most efficient and effective journey through the service. Ensure that people are empowered, connected, independent and have an improved sense of well‑being through the provision of outcome‑focused support. Meet with people regularly to discuss their experiences of the service to drive further improvement.

* Good understanding of personal emotions and behaviours and how they impact on work with citizens and partners; manage own behaviour and emotions to improve personal and professional effectiveness.
