Offer description

We arecurrently growing the team of PCN pharmacists and Technicians within DO&R PCN, therefore we are looking to recruit an enthusiastic clinical pharmacistto deliver excellent support to the practices.

This is avery exciting opportunity to promote self-care.

The successful candidate will have excellent verbal and written communication skills and experience conducting medication reviews.

The post holder will work as part of the multidisciplinary team to developand run processes for repeat prescription re-authorisation and management ofmedicines on the transfer of care.

In return,we will support them in developing their role within the PCN to become non-medical prescriber or gain a postgraduate clinical pharmacy qualification.


Main duties of the job

KEYDUTIES AND RESPONSIBILITIES WILL INCLUDE (please refer to job description attached for further information about each area of responsibility)

Holding patient facing long term condition clinics

Holding patient facing and telephone Clinical Medication Reviews

Undertaking Clinical Medication Reviews for housebound patients within their home

Identifying cohorts of patients at high risk of harm from medicines

Providing face to face and telephone medicines support

Reconciling medicines following patient discharge from hospital, intermediate care or into Care Homes

Providing medicine information to practice staff and patients

Signposting patients to appropriate healthcare professionals

Adhere to repeat prescribing policies

Analyse, interpret and present medicines data to highlight issues and risks to support decision making

Implementation of local and national guidelines and formulary recommendations

Participate in quarterly PCN pharmacy meetings

Take responsibility for own professional development

Please note this post will be based across practices and you may be expected to travel between the practices as well.

Provide expertise in clinicalmedicines review and address public health and social needs of patients in the Practice.

Reduce inappropriate polypharmacy and wasteful prescribing through clinical medication review.

Reconcile medicinesfollowing hospital dischargeand work with patients and community pharmacists to ensure patientsreceive the medicines they need post discharge.

Contribute to reductions inmedicine related hospital admissions and readmissions by supporting patientsto get the best outcomesfrom their medicines and identifying and addressing medicines related issues.

Interface with community andhospital pharmacy colleagues and develop referral processes betweenprimary care professionals including the promotionof the repeat dispensing service.

Oversee and support the dispensary functionof the Practice.

Oversee the repeat prescribingreauthorisation process, including reviewing requests for repeat prescriptions and medicinesreaching review dates.

Manage patients and health care professionals medicine queries.

Increase quality and safety ofprescribing through mechanisms such as audit and PDSA (PLAN DO STUDYACT) cycles.

Manage practice formularies to improve the quality, safety,and cost effectiveness of prescribing.

Implement drug withdrawals and alerts e.g., MHRA, aimedat improving medicines safety.

Work with primary care professionals and patients to implement NICE andother evidence-based guidelines.

Work with Commissioning organisations and SW Healthcare to comply with local medicines optimisation schemesand projects.

Support and hold clinics where medicines are the main interventions e.g., asthma clinics.

Contribute to multi-morbidity reviews and multidisciplinary reviews.

Provide medicines information and trainingto Practice healthcare professionals and adminstaff.

Review daily Pathology resultsfor patients on known medicines.

Act as a source of medicines information for all of the practiceteam and patients (e.g., around doses, side effects, adverse events, possible alternatives e.g., around out of stocks)

Ultimately to undertake minor ailments triage, dealing with minor ailmentsand triaging patientsappropriately.

Working within the practice-based team to undertakemedication reviews particularly in high-risk groupssuch as:

-Frail elderly

-Polypharmacy

-Renal impairment

-Hepatic impairment

-Asthma, diabetes, hypertension and Rheumatoid and Dmard drug monitoring

- Substance misuse

- Patients on high-risk medicines

- STOPP/START identified patients

-Revolving door Hospital admissions

Management support for thedispensary including profitability and stock control; negotiations with drug company representatives andwholesalers; and organisational management including review of SOPS (Standard Operating Procedures) and complianceissues.

Ensure the Practice continuesto participate in, and meet the requirements of, the local DSQS (Dispensing Services QualityScheme)

Improve prescribing practicethrough educational supportfor all prescribers within the practice.

Lead on where changes inevidence require changes in prescribing across the patient population, e.g., where a drug is withdrawnor indications change.

Liaise with colleagues in community pharmacyto align support for medicinesadherence such as MUR (Medication Use Reviews) and NMS(New Medicine Service).

Support improvements in clinicalcare through practice-based audit and implementing change.

Prescribing adviceto prescribers in practicee.g., temporary non-availability of drugs

Ensure patient safety when they are transferred betweencare providers throughreconciliation of prescribed medicines.

Lead for medicines optimisation scheme compliance, includingcare home support;discharge reviews; medication reviews; and QIPP.


Person Specification


Qualifications

* Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council
* Minimum 2 years post graduate experience in Pharmacy, as demonstrated within a practice portfolio
* May be working towards postgraduate pharmacy qualification


Experience

* Has an awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice
* Demonstrates general pharmaceutical skills and knowledge in core areas
* Is able to plan, manage, monitor, advise and review general pharmaceutical care programmes for patients in core areas, including disease states / long term conditions identified by local Pharmaceutical Needs Assessment
* Demonstrates accountability for delivering professional expertise and direct service provision as an individual
* Demonstrates ability to use skills in a range of routine situations requiring analysis or comparison of a range of options
* Recognises priorities when problem-solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
* Is able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
* Involves patients in decisions about prescribed medicines and supporting adherence as per NICE.
