Role Description
This is a full‑time on‑site role as a Care Coordinator located in Newcastle‑Under‑Lyme. The Care Coordinator will oversee and coordinate client care plans, regularly liaise with service users, their families, and the care team to ensure their needs are met. Key responsibilities include managing case files, scheduling, and overseeing the delivery of home care services. The role also involves conducting initial assessments for new clients, resolving customer concerns, supervising staff members, and maintaining compliance with care standards.
Qualifications
* Strong skills in Care Coordination and Case Management for developing and maintaining high‑quality care plans.
* Excellent communication skills to effectively interact with service users, families, and the care team.
* Experience in home care services and delivering tailored support to individuals in their domestic environment.
* Customer service abilities to address client needs while maintaining professionalism and empathy.
* Prior experience in a similar care planning or coordinating role is preferred.
* Knowledge and understanding of care regulations and safeguarding practices.
* Proven organisational skills, with attention to detail and the ability to prioritise tasks effectively.
* Proficiency in relevant administrative tasks, including documentation and scheduling.
