Transitions Support Worker Salary £28,500 per year / 40 hours per week and 1 sleep per month. shifts are 10 or 15 hours ie: 7 am 10 pm. Must be a driver with own vehicle, as will be covering the Bournemouth and Poole areas. ROLE SPECIFICATION Working as part of a multi-disciplinary team to assist the people we support and their families and friends to move their lives in the direction they choose by creative planning and person-centred approaches. The Transitions Support Worker will work closely with and under the supervision and guidance of the Team Managers/Locality Managers and Registered Operations Managers. The Transitions Support Worker may be required at short notice to cover sickness which can include day shifts, evening, waking nights and sleep in support including undertaking transitional support for new people we support. To carry out observational supervisions and catch ups and to carry out Health & Safety and Medication audits. They may be required to undertake on call duties as part of rota that meets the needs of the business and people we support. In addition, the Transitions Support Worker may be temporarily seconded to any support Worker position where needed. Incorporated duties include: To meet regularly with the Team Manager/Locality Managers/Registered Operations Managers, the people and their representative to develop a transition plan that they will implement. To think creatively about all aspects of the persons plan and their life by challenging and encouraging them to consider alternative and new ways of thinking and providing support. To act as the persons advocate where appropriate. Work towards a timeline to support people to move into their own home. To assist the Team Managers / Operational Managers in problem solving around each person we support, whenever this is required. To contribute to the development of Plans, Risk Assessments and Working Policies. To attend meetings and reviews for people. Communicate with and meet others involved in their support, i.e. Care Team Leader, CLDT. The Transitions Support Worker may be required to deputise for Operational Managers absence at meetings. To undertake 24/7 on call duties as and when required on a rota basis. To explore, with the individual and support team, opportunities for accommodation, work opportunities, friendships and hobbies. To support the development of close community connections for the person receiving support. Ensure a high standard of personal care, which may include assistance with personal hygiene, washing, dressing, eating and drinking. Provide practical assistance to the person in carrying out every day living tasks at whatever level they need to enable them to live as comfortably, safely and independently as possible. Tasks may include cooking, gardening, pet care and home maintenance. To take responsibility to alert the Operational Managers of any difficulties within the service and to take any possible action to resolve the problem. Working in a way that also considers the overall financial wellbeing of the organisation. Ensuring that people and their colleagues are safe and that Health & Safety is being considered in relation to each individual being supported. Promote Individuals Health and Wellbeing by: Supporting the person to ensure their primary healthcare needs are met by accessing appropriate facilities for medical, dental, optical treatment etc. Ensure emotional/psychological needs are supported and monitored. Ensure the individuals medication is handled, administered, recorded and stored in accordance with iDirect Independent Livings Policies and Procedures. Enable people we support to take growing control in all areas of their life, taking account of their level of experience, ability and understanding by: Ensuring the people we support are involved as far as possible in decision making which affects them. Ensure that people are involved as far as possible in determining their own routine what, when and how they do things. To work with any person supported by iDirect that may be managed by other Team Leaders, Team Managers, Locality Managers, and Registered Managers where needed. Responsibilities To be available to attend Team Meetings as and when necessary. To ensure mandatory training is kept up to date. Attend all additional training, learning and development opportunities offered. Be responsible for own personal development, including attending supervisions and annual appraisals. In the absence of leadership within the self-managed teams, or close to the people we support, the Transitions Support Worker will take responsibility for decision making and communicating with other Team members. To provide written records/reports as required. QUALIFICATIONS Level 3 Diploma or Equivalent (or willing to work towards) REPORTS TO Team Managers/ Locality Managers/Operations Managers ACCOUNTABLE TO Registered Operations Managers, Head of Operations & Founder/Director HOURS To provide 24 hour waking support which includes unsocial hours, weekends, Bank Holidays, Sleep-ins.
