An opportunity has arisen for a motivated and experienced Band 4 Assistant practitioner to join our Community Mental Health teams. This post will help develop and facilitate interventions and group work programmes to deliver post diagnostic support for people living with Dementia and their families/carers- under supervision of registered staff member
You will be able to work on your own, and as part of a team. You will have experience working with people living with dementia.
You will have the ability to travel across the region based on service need
We will offer regular training, supervision and personal development planning
Posts are 37 hours per week but we would consider part-time or Job share.
