GENERAL PRACTITIONER TEMPORARY MATERNITY COVER

We are a 9 Doctorforward thinking training practice, seeking an enthusiastic individual to joinour committed multidiscipline team

We are currentlylooking to offer a fixed term post for a salaried GP 2-3 days a week (4-6 sessions) for Maternitycover - for an expected 6-12 months duration.

We are situated inmodern premises with onsite pharmacy and ample parking and good commuting linksjust off A470, 15 minutes from Cardiff, close to the modern housingdevelopments and facilities to the west of Caerphilly at the foot of Caerphillymountain.

We have a growinglist size (currently 10,000 patients) and a branch surgery in Senghenydd.

We are a teachingpractice (GP registrars, undergraduate doctors and pharmacists) so we are in agood position to offer support to newly qualified GPs coming out of training.

Currently we useEMISweb and are a paper light practice.

Main duties of the job

Clinical responsibilities:

* Undertaking a variety of duties including surgeryconsultations, telephone consultations and queries, visiting patients athome, checking and signing repeat prescriptions and associated work and correspondence in a timely fashion
* Making decisions in relation to presenting problems,whether self-referred or referred from other health care workers withinthe organisation
* Assessing thehealth care needs of patients with undifferentiated and undiagnosedproblems
* In consultationwith patients and in line with current practice disease managementprotocols, developing care plans for health
* Providingcounselling and health education
* Admitting ordischarging patients to and from the caseload and referring to other careproviders as appropriate
* Recording clearand contemporaneous consultation notes to agreed standards
* Prescribing inaccordance with the practice prescribing formulary (or generically)whenever this is clinically appropriate
* In general thepost-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.
* The postholder will do a proportionate number of On calls

Job responsibilities

Job summary:

Thepost-holder will manage a caseload and deal with a wide range of health needs ina primary care setting, ensuring the highest standards of care for allregistered and temporary patients.

Clinical responsibilities:

* In accordancewith the practice timetable, as agreed, the post-holder will makehim/her-self available to undertake a variety of duties including surgeryconsultations, telephone consultations and queries, visiting patients athome, checking and signing repeat prescriptions and dealing with queries,paperwork and correspondence in a timely fashion
* Makingprofessional, autonomous decisions in relation to presenting problems,whether self-referred or referred from other health care workers withinthe organisation
* Assessing thehealth care needs of patients with undifferentiated and undiagnosedproblems
* Screeningpatients for disease risk factors and early signs of illness
* In consultationwith patients and in line with current practice disease managementprotocols, developing care plans for health
* Providingcounselling and health education
* Admitting ordischarging patients to and from the caseload and referring to other careproviders as appropriate
* Recording clearand contemporaneous consultation notes to agreed standards
* Compiling andissuing computer-generated acute and repeat prescriptions (avoidinghand-written prescriptions whenever possible)
* Prescribing inaccordance with the practice prescribing formulary (or generically)whenever this is clinically appropriate
* In general thepost-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.

Other responsibilitieswithin the organisation:

* Awareness of andcompliance with all relevant practice policies/guidelines, e.g.prescribing, confidentiality, data protection, health and safety
* A commitment tolife-long learning and audit to ensure evidence-based best practice
* Contributing toevaluation/audit and clinical standard setting within the organisation
* Contributing tothe development of computer-based patient records
* Contributiontowards practice evidence based audits, QOF, CEPP, NCN audits
* Deputising forother partners during leave
* Sharing of house calls and paperwork
* Contributing tothe summarising of patient records and read-coding patient data
* Attending andcontributing to weekly partnersmeetings, monthly clinical meetings, bimonthly staff/gp meetings,Quarterly palliative care meetings. Attendance training and eventsorganised by the practice or other agencies, where appropriate.

* In the course ofseeking treatment, patients entrust us with, or allow us to gather,sensitive information in relation to their health and other matters. They do so in confidence and have theright to expect that staff will respect their privacy and actappropriately
* In theperformance of the duties outlined in this job description, thepost-holder may have access to confidential information relating topatients and their carers, practice staff and other healthcareworkers. They may also have accessto information relating to the practice as a business organisation.All such information from any source is to be regarded as strictlyconfidential
* Informationrelating to patients, carers, colleagues, other healthcare workers or thebusiness of the practice may only be divulged to authorised persons inaccordance with the practice policies and procedures relating toconfidentiality and the protection of personal and sensitive data.

Health & safety:

The post-holder will implement and lead on afull range of promotion and management their own and others health and safetyand infection control as defined in the practice health & safety policy,the practice health & safety manual, and the practice infection controlpolicy and published procedures. This will include (but will not be limitedto):

* Using personal security systemswithin the workplace according to practice guidelines
* Awareness of national standards ofinfection control and cleanliness and regulatory / contractual /professional requirements, and good practice guidelines
* Providing advice on the correctand safe management of the specimens process including collection,labelling, handling, use of correct and clean containers, storage andtransport arrangements
* Correct personal use of PersonalProtective Equipment (PPE) and ensuring correct use of PPE by others,advising on appropriate circumstances for use by clinicians, staff andpatients.
* Management of the full range ofinfection control procedures in both routine and extraordinarycircumstances (e.g. pandemic or individual infectious circumstances)
* Hand hygiene standards for selfand others
* Managing directly all incidents ofaccidental exposure
* Management and advice relating toinfection control and clinically based patient care protocols, andimplementation of those protocols across the practice
* Active observation of currentworking practices across the practice in relation to infection control,cleanliness and related activities, ensuring that procedures are followedand weaknesses / training needs are identified, escalating issues asappropriate to the responsible person
* Identifying the risks involved inwork activities and undertaking such activities in a way that managesthose risks across clinical and patient process
* Making effective use of trainingto update knowledge and skills, and initiate and manage the training ofothers across the full range of infection control and patient processes
* Monitoring practice facilities andequipment in relation to infection control, ensuring that proper use ismade of hand cleansing facilities, wipes etc, and that these aresufficient to ensure a good clinical working environment. Lack offacilities to be escalated as appropriate to the responsible manager
* Safe management of sharps use,storage and disposal
* Maintenance of own clean working environment
* Using appropriate infectioncontrol procedures, maintaining work areas in a tidy, clean and sterile,and safe way, free from hazards. Initiation of remedial / correctiveaction where needed or escalation to responsible management
* Actively identifying, reporting,and correction of health and safety hazards and infection hazardsimmediately when recognised
* Keeping own work areas and general/ patient areas generally clean, sterile, identifying issues and hazards /risks in relation to other work areas within the business, and assumingresponsibility in the maintenance of general standards of cleanlinessacross the business in consultation (where appropriate) with responsiblemanagers
* Undertaking periodic infectioncontrol training (minimum twice annually)
* Correct waste and instrumentmanagement including handling, segregation, and container use
* Maintenance of sterileenvironments

Person Specification


Qualifications and skills

* Registration with GMC and locality performers list
* Good communication and interpersonal skills
* Experience in EMISweb, WCCG, AccRx, GPTR
* Specific clinical interests which could enhance the patient care
