This is a new ANP role that allows the successful candidate to work across Hawthorn Medical Centre- Walk-in Service. There is flexibility about which days or hours you work. Our core hours are Monday- Sunday 8am-8pm. The successful candidate's timetable can be flexibly organised around their needs in consideration with the needs of the population at Hawthorn.


Main duties of the job

The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients .

The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.


About us

Hawthorn Medical Centre has aregistered list of 6500+ patients and an unregistered walk-in element that sees 17000 patients a year.

We are looking for ANPs to join our established team of ANPs and GPs that run the walk-in service. Each day is different, so you need to have great diagnosis skills and be able to think on your feet. You will be part of a team that provides support, training, and a good amount of tea and cake! You will also be joining the wider team of ANPs across the company who get together annually for learning and development opportunities.

Hawthorn Medical Centre is part of the Hope Citadel Healthcare, a team of 11 practices that are looking to bring transformation in individuals, families and communities through whole-person centred healthcare.


Job responsibilities

Communication

Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communication

Utilisecommunication skills to support patients to adhere to prescribed treatmentregimens

Delivering aquality service

Recognise andwork within own competence and professional code of conduct as regulated by theNursing and Midwifery Council (NMC)

Produce accurate,contemporaneous and complete records of patient consultation, consistent withlegislation, policies and procedures

Deliver careaccording to the NSF and the National Institute for Clinical Excellence (NICE)guidelines and evidence-based care

Utilise the auditcycle as a means of evaluating the quality of the work of self and the team,implementing improvements where required

Participate inthe management, review and identify learning from patient complaints, clinicalincidents and near-miss events utilising a structured framework (eg root-causeanalysis)

Team Working

Work as aneffective and responsible team member, supporting others and exploring themechanisms to develop new ways of working

Delegate clearlyand appropriately, adopting the principles of safe practice and assessment ofcompetence of those taking on delegated duties

Managementof Risk

Manage and assessrisk within the areas of responsibility, ensuring adequate measures are inplace to protect staff and patients

Monitor workareas and practices to ensure they are safe and free from hazards and conformto health, safety and security legislation, policies, procedures and guidelines

Ensure safestorage, rotation and disposal of vaccines and drugs is undertaken.

Undertakemandatory and statutory training

Use technology asan aid to management in planning, implementation and monitoring, presenting andcommunicating information

Review andprocess data using accurate Read codes about patients in order to ensure easyand accurate retrieval for monitoring and audit processes

Learning andDevelopment

Act as mentor tostudents, assessing competence against set standards as requested and ifappropriately qualified

Make effectiveuse of learning opportunities within and outside the workplace, evaluatingtheir effectiveness and feeding back relevant information

Person Specification


Knowledge

* Aware of accountability of own role and other roles
* Awareness of clinical governance issues in primary care
* Knowledge of patient group directions and associated policy
* Awareness of the concept of whole person healthcare.
* Acute ailments management
* Ability to identify determinants on health in the local area
* Knowledge of public health issues in the local area
* Awareness of local and national health policy
* Awareness of issues within the wider health economy


Qualifications

* Registered First level Nurse
* Clinical supervision and training experience


Skills

* Clinical skills around diagnosis and treatment of minor ailments
* Communication skills, both written and verbal
* Ability to communicate difficult messages to patients and families
* Negotiation and conflict management skills
* IT skills in clinical systems such as EMIS Web


Experience

* Minimum 2 years post-registration experience
* Experience in Primary Care
* Involvement in implementing and using protocols and clinical guidelines

£55,000 to £65,000 a yearDepending on experience
