Job overview

The Care Home Liaison team (CHLT) offers assessments and interventions to support patients dealing with various mental health issues, including dementia, residing in care homes.

This service is accessible to patients over 65 living in residential care or nursing homes, and those under 65 diagnosed with dementia.

The Band 7 Clinical Nurse Specialist will provide advanced clinical expertise, leadership, and consultancy to care homes within the North Lincolnshire geographical area. The CNS will work in partnership with care home staff, GP’s, primary care networks, community services, mental health teams, and acute trusts to support high-quality, person-centred care for older people.

The post holder will play a key role in reducing unnecessary hospital admissions, supporting timely discharges, and improving outcomes for care home residents through education, clinical support, and service development.


Main duties of the job

* Deliver specialist, evidence-based assessment and management of individuals under the services care with complex health needs, including frailty, dementia, and long-term conditions in accordance with NICE guidance.
* Provide advanced clinical advice and support to care home staff to enhance their knowledge and skills in managing patient’s health and wellbeing
* Lead on developing, implementing, and evaluating individualised care plans in collaboration with multidisciplinary teams
* Act as a clinical leader and role model, promoting high standards of care and best practice across care homes.
* Provide specialist input to case discussions, safeguarding concerns, and multidisciplinary reviews
* Support care home staff with complex decision-making and ethical issues, including capacity assessments and advance care planning.
* To determine management plans, and where appropriate, deliver short term psychological treatments, for people who are referred with complex mental health needs based upon a comprehensive psychosocial, risk and needs assessment of that individual.
* To represent the Trust from a Mental Health Perspective and attend relevant meetings for your role to promote collaborative working and build on working relationship with internal and external stakeholders.

Person specification


Qualifications

* Registered Mental Health Nurse
* Evidence of other development and/or training which indicates commitment to continuous development and preparation for leadership
* ENB 998 or other NMC approved mentor qualification or mentor qualification appropriate to discipline
* Specialist training within Older Adult Mental Health
* Nurse Prescribing or willingness to complete within an agreed timescale
* Management or leadership qualification and/or training


Experience

* Demonstrable post registration experience at a Band 6 level within an assessment or liaison service
* In depth experience in the effective and positive management of complex and high risk cases
* Previous experience of working autonomously to make comprehensive decisions
* Experience of MDT working
* Skills in coordinating and planning inter-agency approaches in mental health care
* Experience of delivering evidence based treatments
* Experience of leadership and delivering supervision to junior members of staff
* Involvement with service development and change management initiatives


Knowledge

* Ability to use initiative and work without immediate supervision
* Ability to clinically lead, supervise and direct a team and support staff as appropriate
* Ability to prioritise workload and manage team capacity according to demand
* Ability to demonstrate a flexible, adaptable attitude to meet complex client needs
* An understanding of current mental health issues relating to the provision of Mental Health Liaison Services
* Demonstrate a thorough knowledge, understanding and practical experience of risk assessment and risk management
* Knowledge and understanding of the legislative framework for mental health services, particularly the Mental Health Act and Mental Capacity Act
* Demonstrate excellent problem solving and conflict resolution skills
* Demonstrate an ability to make balanced clinical judgments and decisions
* Demonstrate an ability to cope with the expectations, pressures and demands of this post
* Demonstrate an ability to work collaboratively and respectfully with colleagues from a range of different professional backgrounds and across professional and organisational boundaries

* Demonstrate excellent communication skills, both written and verbal
* Skills in co-ordinating and planning interagency approaches in mental health care
* Demonstrate specialist knowledge and skills relating to the post
